Sample Supports is looking for a LGBTQ+ supportive Host Home Provider to share their home with a female individual in services transitioning from an independent living setting. This individual needs a provider who is supportive of the LGBTQ+ community and who is willing to provide support in navigating personal and professional dynamics that have developed as being a member of this community. The best fit provider should be familiar with LGBTQ+ medical needs and share an openness and willingness to learn about how these impact the individual.Β 


This individual has a wonderful sense of humor and would benefit from a home that they feel socially included in. The individual utilizes the electric wheelchair on occasion in the community and more frequently utilizes the walker. The individual is independent in managing the maintenance of their assistive devices. Other common needs of this individual include support with medication management, scheduling and maintaining appointments, and encouragement to engage in community and service related activities.


We welcome Host Home Providers of all experience levels that have at least one private bedroom available in their owned or rented home. People who have experience with disabilities and/or caring for adults that are aging either in their own families or professionally fit this role organically.


Host Home Providers are independent contractors with us and receive competitive typically tax-free payments based on the difficulty of care required for the person. Sample Supports provides comprehensive training and certifications plus specific training to the individual in care. Our residential team is on-call 24/7 to support you as needed.Β

How you will support this individual in your home:Β 

  • Provide a LGBTQ+ safe-space in your home.
  • Be supportive of the individual's chosen identity including providing transportation to and support at LGBTQ+ affirming doctors appointments and medical treatments.
  • Encourage independence while providing support with tasks.Β 
  • Complete medication management by holding and administering all medication.
  • Support the individual in maintaining a clean and safe living environment by prompting cleaning and maintaining expectations. May need to provide direct support with this on a less frequent basis.
  • Prompts for individual to complete activities of daily living including hygiene tasks.
  • Complete training and follow details of individual behavior support plan (IBSSP) including interventions for common behaviors such as refusal to engage in required tasks, verbal aggression, fixation on various concerns or worries, and statements of self-harm.
  • Evening/overnight Β in-home support with medication administration Β (individual has unlimited unsupervised time)

General Provider duties and responsibilities:

  • Provide personal assistance, social and emotional support, and other personal care to the individual in a home and community environment.
  • Support with activities of daily living including shopping and meal preparation, personal care/hygiene routines, and general in-home support tasks as needed
  • Assisting in planning and providing transportation to community activitiesΒ 
  • Transportation services as needed - may include transportation to/from scheduled activities, work and appointments
  • Complete documentation for services provided using electronic systems
  • Medication administrationΒ 

Compensation:

Compensation is typically tax free. $3,066 - $3310.80/month depending on location. Supplemental room and board payments may also be provided directly from the individual at up to $779/month.Β

  • Must be at least 18 years old
  • Must be LGBT+ friendlyΒ 
  • Must be able to lift at least 75 pounds
  • Must have dependable and reliable transportation with the ability to safely transport
  • Must have a valid driver's licenseΒ 
  • Must meet approved motor vehicle record standards for providing transportation for individuals in servicesΒ 
  • Must be able to pass Child/Adult Protection Services checkΒ 
  • Due to the nature of the work performed at Sample Supports, and that this position works with an at risk population, we do require a criminal background check be completed.
  • Must have appropriate living environment with at least one private bedroom available for placements
  • Must be willing to perform Non-Violent Crisis Intervention physical managements as trained
