Job purpose:

A Community Inclusion Specialist provides an array of supportive services to individuals with disabilities in fully experiencing their community. Β  Supportive services may occur individually or within a group setting, offering robust experiences at work or in the community. Β  The Community Inclusion Specialist is pivotal in creating enriching and dynamic experiences for individuals to lead fully inclusive lives within their communities and with people whom they choose to engage with.Β

Duties and Responsibilities

  • Develop community connections that will offer dynamic, interactive, educational, and exciting experiences for individuals to have inclusive relationships with others in the community.
  • Promote individualized and group experiences that will be mutually beneficial to individuals and the community at large.
  • Provide on the job support for individuals who are engaged in employment, encouraging positive and professional relationships with employer and coworkers while fostering high quality work performance.
  • Offer guidance and suggestions on post-employment leisure and personal development activities.
  • Record and maintain individuals’ records in Therap as assigned (data entry, case notes, updated contacts, and information). Β 
  • Collaborate with other departments and services within the organization to cultivate new experiences and opportunities around inclusive experiences.Β 
  • Socialize and demonstrate the core values of the organization
  • Lead self-advocacy discussions and family engagement experiences to foster future growth and development.
  • Develop and offer dynamic and interactive activities that support personalized goals, group desires, and pathways for independence.
  • Develop peer relationship opportunities while fading supports and promoting natural experiences.Β 

Qualifications:

  • High School Diploma.
  • One year of experience working with persons with disabilities.
  • Valid Driver’s License and must maintain a satisfactory driving record.
  • You are required to be in possession of a car on site during your entire shift to transport yourself and/or individuals served to community sites in all weather conditions and throughout all neighborhoods.
  • Must have computer/technology capabilities that includes use of internet and Microsoft Office Tools.
  • Work a flexible schedule that may include early mornings, evenings, weekends and/or holidays.
  • Work independently, utilize good judgment, and possess a strong sense of initiative.

Physical Requirements:

  • Depending upon assignment, must be able to work in a variety of settings, weather conditions and locations.
  • May provide hands on personal care which may include toileting, feeding, cleaning and all other activities of daily living skills.
  • May provide physical interventions which may include lifting, mobility, first aid, and safety evacuations.
  • Move and lift weights up to 20 lbs. on an occasional basis.
